This online tool, published by the Social Care Institute for Excellence, aims to help you achieve better integrated working and in doing so deliver quality care and support around the needs of the individual, their family and their carers.
The tool will help you to establish effective professional relationships with colleagues across sector and service boundaries.
At the heart of the tool is a series of questions that can be used to facilitate your conversations. The questions are grouped around organisational, cultural and professional, contextual and co-production issues. The questions will enable you to:
- Reflect on the degree to which your own arrangements and practices promote effective integrated working
- Decide if you need to do anything differently – and if you do, the questions will help you consider what you need to do and how you can do it
The questions are supported by short video sequences of people who use services talking about their experience of joint working. The tool also has videos of practitioners and managers discussing how to meet the challenges associated with this way of working. The videos are designed to promote your own reflection.
When you’ve worked through the questions, you’ll come away with a document that you can use to improve your integrated working.
